Community Data Survey

Introduction

The Haliburton, Kawartha, Pine Ridge (HKPR) Distict Health Unit is collecting information from you in order to understand drug poisoning and overdose activity happening in the Haliburton County, City of Kawartha Lakes, and Northumberland County.    The information you provide through this survey will be used by the HKPR District Health Unit to help understand and respond to drug poisonings and overdoses in our communities.

The survey should take less than 10 minutes to complete, but please take as much time as you need to give your answers.

Your participation in completing this survey is completely voluntary. All responses are anonymous and cannot be linked back to you. All information collected will be stored electronically and password protected in a safe location.

You can choose to stop the survey at any time or skip any question(s) that you are uncomfortable answering. Your decision to complete or not complete the survey will not have any impact on the services you receive from the HKPR District Health Unit.
