HKPR District Health Unit COVID-19 Mass Immunization Clinic Survey

Help us to improve our COVID-19 mass immunization clinics! Complete the survey if you were vaccinated at a Health Unit clinic, OR attended as a caregiver/support person.

A HKPR District Health Unit COVID-19 mass immunization clinic is:
  • held at a public location such as a community centre
  • open by appointment to members of the public
  • coordinated by the HKPR District Health Unit
  • located within the HKPR District Health Unit region (Northumberland County, Haliburton County, City of Kawartha Lakes)
Vaccinations that occur at other settings, such as pharmacies and care homes are not included.
  • The survey is open to anyone who has been vaccinated at, or has been a caregiver or support person to someone while they were vaccinated at a HKPR District Health Unit COVID-19 mass immunization clinic. 
  • The survey is open now.
  • Survey will stay open until two weeks after all the mass immunization clinics have closed.
  • Your answers are anonymous and confidential.
  • You will not be identified with any of the responses you provide.
  • If there is a question that you would rather not answer, you can leave it blank.
  • Your participation is voluntary, and you may quit the survey at any time. Your decision will have no impact on your participation in services or programs provided by the Health Unit.
  • Information will be stored electronically, and password protected in a secure location.
