Hamilton Public Health Services is inviting you to participate in this online survey. For participating you will be entered into a draw for one of three $25 gift cards to Walmart or Amazon. Your participation will take about 10 minutes. The feedback we are looking for is about our Child & Adolescent Services. You will be asked about the logistics of attending appointments, including availability, locations, things that make it harder and easier to attend appointments. The survey is voluntary and confidential. The final report may be shared with other community programs to inform and improve services for families. Whether you participate or not will not impact current or future Public Health Services you receive.
For any written responses please do not include any information that can identify you, like your name, phone number, etc. For the gift card draw you will be directed to a separate survey with a link to send a pre-populated email to indicate your inclusion in the gift card draw. This won’t be connected to your survey responses in any way and your email address will only be used to contact you if you are a winner of the gift card draw.
