Sexual Health Services Survey

Lakelands Public Health wants to hear about your experience with sexual health services.

This survey is confidential. All information collected will only be used by Lakelands Public Health.  

We want to know about sexual health services in our region. This includes: 

  • Lakelands Public Health clinics  

  • family doctors 

  • community health centres 

  • walk-in clinics 

  • hospitals  

We are asking about all sexual health services in our region, not just Lakelands Public Health clinics. This information will help Lakelands Public Health understand community needs. It will also help us improve our sexual health services.

 

Your experience matters 

We want to hear from you if you meet both of the following: 

  • You have used, tried to get, or needed sexual health services in the past 12 months, and 

  • You live, work, or go to school in one of these areas:  

                  - County of Haliburton  

                  - City of Kawartha Lakes  

                  - Northumberland County

                  - Peterborough County, including the City of Peterborough

 

Before you start 

Please read this before starting the survey: 

  • Your survey answers will be kept confidential.  

  • Taking part in this survey is your choice. You do not have to complete it. 

  • You can choose not to take part or stop at any time. This will not affect the services you receive from Lakelands Public Health. 

  • You can skip any question you do not want to answer. 

  • This survey takes about 20 minutes to complete. 

  • Your survey answers will be stored electronically in a secure, password-protected system. No information that identifies you will be included in reports or presentations. 

  • Lakelands Public Health will use the survey findings to plan, evaluate, and improve our sexual health services. Findings may be shared in summary reports and presentations within Lakelands Public Health.

 

Thank You (Gift Card Draw) 

As a thank you for your time, people who complete the survey can enter a draw to win one of four $100 gift cards. Winners can choose their own gift card from over 100 merchants and receive it electronically or by mail. 

If you choose to enter the gift card draw, you will be redirected to a separate survey after completion where you can provide your email address. Your email address will be stored securely and kept separate from your survey answers. Your email address will not be linked to your responses.
